The film is based on the real-life trial of Arne Cheyenne Johnson, a man who claimed to have been possessed by a demon when he committed murder. The story begins with paranormal investigators Ed and Lorraine Warren attending the exorcism of David Glatzel, a young boy who is believed to be possessed by a demonic entity. During the exorcism, Arne, David's sister's fiancé, taunts the demon to leave David and possess him instead. Following the exorcism, strange occurrences continue to plague the Glatzel family, and Arne begins to exhibit signs of possession himself.

One night, Arne becomes fully possessed and brutally murders his landlord under the demon's influence. Ed and Lorraine, who were present during the exorcism, are convinced of Arne's innocence and seek to prove that he was under demonic influence at the time of the crime. As the Warrens investigate further, they uncover a sinister conspiracy involving a cult that has been practicing dark rituals and invoking demonic entities. They also discover a cursed totem that serves as a conduit for the demon's power and is linked to Arne's possession.

The Warrens race against time to find evidence to exonerate Arne and stop the demonic forces at work. They encounter numerous obstacles, including skepticism from law enforcement and opposition from the cult, who seek to protect their dark secrets at any cost. In a climactic showdown, Ed and Lorraine confront the demon in a remote underground tunnel, where they perform a ritual to banish it back to Hell. With the help of their faith and determination, they manage to free Arne from the demon's grip and bring an end to its reign of terror. The film concludes with Arne standing trial for the murder, with Ed and Lorraine providing testimony about the supernatural events surrounding the case. Despite the challenges they faced, justice is served, and Arne is acquitted of the crime, thanks to the Warrens' efforts.
