"Old" opens with the Cappa family-Guy, Prisca, and their two children, 11-year-old Maddox and six-year-old Trent-embarking on a tropic vacation to reconnect. Such a program has been put into place because of latent tension between them: Prisca's health issues and her resolution to separate from Guy. The family finally arrives at an exclusive resort that hints very well: warm greetings from the resort staff hint at one thing very different and much in store for them in this getaway from the world. The manager recommends a private beach in some seclusion for them, and they gladly take up the offer.
The next morning, the Cappa family and some guests-Mid-Sized Sedan, a rapper; Charles, a surgeon; Chrystal, his wife; Kara, their little daughter; Agnes, the mother of Charles; Patricia, a psychologist; and her husband, Jarin-are brought to the mystic beach. The setting, while beautiful, is inaccessible due to steep cliffs at either end of it. Some unnerving incidents start to occur shortly after their arrival. In a few hours, the body of Mid-Sized Sedan's companion is found, decomposing rapidly. Agnes collapses and dies suddenly, yet another big surprise. Attempting to understand what's going on, the guests note some bizarre changes: the children are aging at a fantastic rate. Maddox and Trent now appear to be teenagers, and Kara looks distinctly older. They start to panic when they find out time on the beach comes at a weirdly fast clip; every half hour equates to years of aging. But as the group struggles to come to grips with what has occurred to make them prisoners, squabbles rise to the surface. Charles's grip on reality slackens, and he begins swinging between paranoia and violence. Prisca and Guy desperately try to shield their children from emotional and physical pressures brought on by this rude awakening into adulthood. The attempts at leaving the beach end in some sort of invisible barrier created by the cliffs and surrounding water that seems to disorient them and always prevents them from getting out.

Secrets spill in the midst of anarchy: Prisca reveals an affair to further strained her marriage with Guy, Charles' paranoia peaks into violent outbursts, Chrystal breaks down as her body begins to show the actions of time, Patricia's long-dormant epilepsy starts acting up again, and Jarin frantically attempts to find a means off the beach. The situation becomes graver when the guests start dying one by one due to the fast aging process and other medical conditions. Grown-up Kara falls to her death while trying to climb the cliffs. Paranoia drives Charles into a fatal fight with Guy, who fights in defense of his family. Chrystal dies of brittle bones from calcium deficiency, Patricia and Jarin die due to their conditions.
The large number of people that were there in the beginning dwindles down to just the Cappa family as night falls. Guy and Prisca finally make amends, realizing how pointless the fights in their past have been. By morning, they are old and the time they have left with each other slips away. The two share one peaceful moment together before dying, leaving their children at middle age, Maddox and Trent, alone.
The siblings, determined to live through their experience, manage to piece together the evidence that had been left behind by the previous visitors of this beach. It is discovered that the resort has been carrying out unethical experiments on its guests visiting this beach, using the time-disorting characteristics of the beach for testing new drugs for various medical conditions. With this in mind, the siblings find a way out of the beach through an underwater coral tunnel. Hazardous it is, they manage to break themselves free. Maddox and Trent head back to the resort and expose the brutally honest truth to the authorities. The authorities shut down the resort and arrest the people responsible. However, even though their parents were gone and their childhood had been ruined, Maddox and Trent found comfort in each other, making a resolution to rebuild, knowing full well that their parents' love and sacrifice had paved the way.
