The story begins in the 15th century with Vlad III, Prince of Wallachia, portrayed by Luke Evans. Vlad, also known as Vlad the Impaler, is a respected leader who has returned to his homeland after serving as a hostage for the Ottoman Empire. He seeks to maintain peace and protect his people from the Ottoman threat. However, his peaceful intentions are disrupted when Turkish emissary Mehmed II (Dominic Cooper) demands a thousand Wallachian boys, including Vlad's son, for the Ottoman army.

Desperate to protect his family and his people, Vlad turns to a mysterious mountain cave, home to a vampire (Charles Dance) who grants him dark powers. Vlad drinks the vampire's blood, gaining superhuman strength, speed, and other supernatural abilities. He is given an ultimatum: resist his newfound thirst for blood for three days, or fully succumb to the vampire curse.


As Vlad grapples with his newfound abilities, he discovers that he can transform into a swarm of bats and manipulate the elements. Faced with the Ottoman threat, he decides to embrace his vampiric powers to defeat Mehmed's forces and save his people. The three-day deadline looms over him, adding urgency to his mission. Vlad's transformation is both physical and psychological. He becomes a formidable force on the battlefield, using his powers to decimate the Ottoman army. However, the cost of his actions becomes apparent as he struggles with the morality of his choices and the impact on his humanity. Meanwhile, his wife, Mirena (Sarah Gadon), begins to suspect the changes in Vlad and fears losing the man she loves.

As Vlad's powers grow, so does his reputation as a supernatural entity, leading to fear and superstition among his own people. Mehmed, determined to conquer Wallachia, discovers Vlad's secret and unleashes an assault on Vlad's castle. The confrontation between Vlad and Mehmed reaches its climax, with Vlad ultimately defeating the Ottoman forces.


However, victory comes at a great cost. Mirena is mortally wounded during the battle, and Vlad realizes that he cannot save her without turning her into a vampire. Refusing to doom his wife to an eternity of darkness, Vlad decides to sacrifice his own immortality to bring her back to life.

In a heartbreaking twist, Vlad's sacrifice means his demise at the hands of Mehmed's soldiers. The movie concludes with Mirena mourning Vlad's death but discovering that he lives on in another form. The vampire who granted Vlad his powers appears, revealing that Vlad has become the new protector of Transylvania and a guardian against supernatural threats.
